ValorPath Veteran Care Access provides non-clinical coordination and access services that support how veterans connect to hospice and palliative care after VA Community Care authorization. We do not deliver medical care. Our value is in referral execution, network coordination, and Veterans First contracting alignment.

  • Referral Execution Coordination
    We support the referral process by coordinating the execution of authorized care, routing veterans to appropriate, compliant hospice and palliative subcontractors based on capacity, coverage, and readiness.
  • Subcontractor Network Governance
    ValorPath Veteran Care Access maintains a credential-ready subcontractor network of Medicare-certified hospice and palliative care providers. We track compliance documentation, readiness, and performance standards to support efficient onboarding and consistent service delivery.
  • Performance & Responsiveness Oversight
    As an SDVOSB prime platform, ValorPath Veteran Care Access provides single-point accountability for referral execution, including response expectations, escalation pathways, and performance visibility across our subcontractor network.
  • Veterans First Contracting Alignment
    ValorPath Veteran Care Access’s structure as a Certified SDVOSB & VOSB (SBA VetCert) enables aligned organizations to operationalize Veterans First contracting priorities in hospice and palliative access without changing clinical workflows.
