How ValorPath Veteran Care Access Supports VA Community Care

ValorPath Veteran Care Access strengthens veterans’ access to hospice and palliative care by supporting execution and accountability after VA authorization without replacing VA systems.

VA Community Care authorizes and oversees all clinical care; ValorPath does not provide medical services. As an SDVOSB prime coordination platform, we manage referral execution through a vetted network of Medicare-certified hospice and palliative care subcontractors.

We provide:

  • Network governance across compliant subcontractors

  • Performance visibility and escalation support

  • Veterans First alignment through our SDVOSB/VOSB structure

This model preserves full VA clinical control while delivering structure, consistency, and national reach through a veteran-owned platform.
